Only referral partner organizations who have submitted an Application and Partner Agreement to Bottomless Closet may refer clients. If you are unsure if your organization is a partner agency please check the list of current agencies here or contact Patrycja Warda. If you are a staff member who has attended orientation at Bottomless Closet you may make direct referrals. If you have not attended orientation, please register for the next session prior to making a referral.

	Pre-Interview Appointment Request Form Please use this form to refer a client who has an upcoming job interview scheduled
	Post-Employment Request Form Please use this form to refer a client for a follow-up visit after she has been employed
	Statement of Understanding Please give this form to your client to read, sign, and bring with her to her initial appointment at Bottomless Closet
